Traditional Hair Transplantation vs. Follicular Unit Extraction

There are two primary types of hair replacement surgery – a procedure commonly referred to as the “strip method” and a newer procedure called follicular unit extraction (FUE).

For years, hair transplantation has been performed using hair from a strip of skin that is removed from back of the patient’s head (the “donor” area). While this procedure produces good results, it results in a linear scar across the back of the head, which is visible when the head is shaved or the hair cut very short.

FUE, Dr. Siporin explains, represents a breakthrough in hair transplantation. FUE is performed using a small round instrument to remove hair follicles one at a time. FUE can be used to harvest the same amount of hair as can be harvested via a strip procedure, but leaves behind no visible scar. Dr. Gregory Herbich of the Cosmetic & Laser Surgery Center says that in order to minimize scarring, his office typically will do breast augmentation through the bellybutton with a procedure called Transumbilical Breast Augmentation or TUBA.  “I’m pretty happy with that procedure,” says Dr. Herbich, “because it seems to be less painful than the other ways to do it and of course there are no scars on the breasts; and if you have to remove the implants the breasts still look normal.”  He says breast lift operations may differ from straight augmentation using the TUBA surgery when there’s too much skin or if a patient would prefer to achieve lift without an implant.  In the case of excess skin, Dr. Herbich typically would place an implant and then excise skin around the nipples in order to reduce scarring on the breasts.  If a patient wants to achieve lift without an implant, the result from breast lift around the nipple is not quite as good, says Dr. Herbich, “for those patients we do a more traditional mastoplasty with a lollipop or anchor-shaped scar.”

The Initial Consultation

Risks associated with Honolulu breast augmentation surgery are a little different from those associated with breast lifts, says Dr. Herbich, in that there is a foreign body there, so the main additional risk is increased possibility for infection and hardening of the breast.  In general, complications from surgery are “not common, but not rare either,” according to Dr. Herbich.  The risks may include: Scarring, bleeding, infection, capsular contracture—hardening from scar tissue that forms around the implant, pain following surgery, rupture/deflation of a saline implant, rupture of a silicone gel implant, altered nipple sensitivity, changes in nipple direction, increased droopiness of the breasts, and stretch marks.  Some of the complications can lead to additional risks and others can be treated with additional cosmetic procedures, which is why it’s important to consult a highly-qualified Honolulu cosmetic surgeon who will be with you through the whole process.

The Recovery

Breasts usually look pretty good about a week after implant surgery, but full recovery usually takes about six weeks and Dr. Herbich recommends not returning to full exercise until that time.  The recovery time for breast lift surgery is similar, at around four to six weeks.
