The process of planning for breast augmentation surgery begins with understanding your goals for the procedure. Are you looking to increase the size of your naturally small breasts? “There are a lot of nuances involved in breast augmentation,” says Dr. Jandali. “And developing a treatment plan involves first listening and really understanding what the patient is asking for.”

Silicone versus Saline Implants

One of the first decisions you’ll need to make about your breast augmentation procedure is whether you’ll receive silicone or saline implants. Some women are uncomfortable with the idea of silicone implants, primarily because of the concern that they may rupture and cause harm.

Studies have shown, however, that silicone implants are very safe, and for some patients will produce a much more satisfactory result. A very thin person, for instance, should consider silicone implants as they will produce less “rippling” and will feel more natural. Your doctor will discuss with you the differences between saline and silicone implants and which may be best for you.

Implant Placement

The placement of your implants – whether above or below the pectoralis muscle – is another key consideration. For most women, placement below the muscle will provide the best result.  This placement provides an extra layer of cushioning, reduces the risk of hardening around the implant over time, and in very thin women can help prevent rippling. Bodybuilders or very athletic women will benefit more from placement above the muscle.

Incision Location

Your doctor will also discuss your options in terms of the placement of your surgical incisions (i.e., where the implants will be inserted). The most popular location is beneath the breast. This type of incision is well hidden by the breast itself, and doesn’t require the surgeon to work through breast tissue in order to insert the implant.

Another option is an incision around the nipple. This kind of incision is best for breast augmentation that also involves a breast lift. Alternatively, the incision can be placed in the armpit.
